Facility Rental Request Form

Rental bookings are not confirmed until we receive the Facility Rental Agreement (signed by the Licensee), payment, insurance, and damage deposit, if required.
Official in charge must be 19 years or older and an executive officer of the group.
Coaches/group leaders, etc. are to make requests through their executives.

Insurance
All user groups must either provide a copy of their Liability Insurance policy, which must indicate the following:
  • “Board of Education, School District No. 33 (Chilliwack)” is named as an Additional Insured
  • Minimum of two million in liability coverage
Or may purchase insurance coverage through the District or local insurance provider; the charges will be determined once information pertaining directly to the rental has been provided.
