Facility Rental Request Form

Please let us know what Organization you are affiliated with. 

What would you like the event to be called? (this can be changed later).

Tell us a little bit about your event. Example: Soccer Practice for youth/adults, Strata Meeting, etc. Is there going to be fees to attend your event etc. 

Is there a fee to attend your session/classes? If yes, please give us some more detail. If no, just put "No".

Please specify if you currently have insurance.

Please enter date(s) you would like to book. If you would like to rent for more than one day please specify how many dates you are looking to book and what your start and end date would be.  

What time would you like to have access to the room for set up etc. (ex. if your meeting is 530pm - 730pm but you would like to have half an hour for set up and take down, then your access and exit time would be 5pm-8pm)

What time does your event/meeting start and end. 

Please let us know if there is a specific site(s) that you are wanting to rent? (ie. NLC Alumni Hall, Vedder Elementary Gym...etc). If you are not sure what site you would like to rent then please tell us what type of space you are looking for. 

Please specify what type of room you would like. Such as:

Please let us know how you would like tables and chairs set up and what type of equipment you will need. ie. AV equipment.
